Rethinking placebo in psychiatry: how and why placebo effects occur†
نویسنده
چکیده
The range of placebo mechanisms can be bewil dering, but evolution provides an overarching theoretical framework that permits the integration of the different levels of explanation. Tinbergen’s (1963) classification, for example, divides the causes of behaviours such as placebo responses into proximate mechanisms and ultimate mechan isms (Box 1). This framework allows the integration of physiology, neuroscience, social and developmental psychology, psychodynamics, anthropology and evolutionary biology. We will start by considering the proximate mechanisms, which are subdivided into: causa tion, which includes immediate psychological and physiological stimuli, meanings, learning and the neurochemical substrate; and development (ontogenesis) – how behaviour is influenced by early experience and how it changes with age (Fig. 1).

Rethinking placebo in psychiatry: the range of placebo effects†
The term ‘placebo’ is used in different contexts with different meanings (Box 1). Placebo effects have been a scientific curiosity for years, regarded as deceptive therapies and tainted by associations with quackery and dishonesty. Anthropological studies of the placebo (Thompson 2010) highlight the universality of charms and ritual to produce healing effects in all cultures, suggesting an evol...

Background: Given the potential role of the 5-hydroxytryptamine-3 receptor in the pathogenesis of schizophrenia, this study was performed to determine whether ondansetron plus risperidone could reduce the negative and depressive symptoms in patients with treatment-resistant schizophrenia. Methods: In a double-blinded, placebo-controlled, randomized trial (IRCT registration # 201112125280N7), in ...
متن کاملPlacebo interventions, placebo effects and clinical practice.
This article reviews the role of placebo interventions and placebo effects in clinical practice. We first describe the relevance of different perspectives among scientists, physicians and patients on what is considered a placebo intervention in clinical practice. We then summarize how placebo effects have been investigated in randomized controlled trials under the questionable premise that such...

Background and Objective: Considering the hypothesis of the effects of the use of antioxidants such as ascorbic acid on the improvement of oxidative stress induced by schizophrenia, the present study was designed to investigate the effect of ascorbic acid as an adjunctive therapy with risperidone in controlling the symptoms of schizophrenia. Materials and Methods: This randomized clinical tria...
متن کاملWhy the Critics of Poor Health Service Delivery Are the Causes of Poor Service Delivery: A Need to Train the Policy-makers; Comment on “Why and How Is Compassion Necessary to Provide Good Quality Healthcare?”
This comment on Professor Fotaki’s Editorial agrees with her arguments that training health professionals in more compassionate, caring and ethically sound care will have little value unless the system in which they work changes. It argues that for system change to occur, senior management, government members and civil servants themselves need training so that they learn to understand the effec...
